For New Patients

Bring your insurance card, a referral (if needed), completed new patient forms, any paperwork from your referring physician, and any recent X-rays. Please download the New Patient Form, print the three pages on your home printer, complete the form, and bring it to the office for your first visit. Our urologists are providers for most major insurance programs. Please check your insurance’s list of participating physicians to be sure your program allows you to be seen at our office. Be aware that you may have out-of-network benefits that will allow you to see our physicians. Our billing experts are available to assist you with any questions or concerns.
